Duties & Responsibilities:

Serve as the primary client contact for care coordination and EHR documentation
Conduct screenings and needs assessments
Coordinate referrals and appointments
Monitor engagement and follow-up
Provide psychoeducation
Connect clients to community resources
Maintain HIPAA compliance.

Requirements:

Masters (or foreign equiv. deg.) in Clinical Mental Health Counseling, Psychology, Social Work or rel. + knowledge of mental health conditions, treatment systems, EHR systems.
